TELEPHONE SERVICES LONG DISTANCE TERMS AND CONDITIONS

I request access to the Telephone Services (TS) long distance service. By signing the registration form, I agree to the following:

1. AUTHORIZATION CODE: I will be assigned my own individual Authorization Code which will enable me to place long distance calls. I understand that my Authorization Code is for my use only, and I will be responsible for all calls placed with my code. It is prohibited to share my Authorization Code. If I do share my code, I will be responsible for all charges incurred. I am only to use the Authorization Code assigned to me. Use of any Authorization Code other than my own will constitute abuse. I will be responsible for any fees associated with the investigation of such abuse, and I will be liable for payment of any calls. If I should lose my Authorization Code, it is my responsibility to notify the TS office immediately. I will be responsible for all calls made until I notify the TS office. Upon receipt of notification, TS will deactivate my existing Authorization Code and issue me a new code. A service fee will be assessed for lost codes.

2. PAYMENT: I understand that use of my Authorization Code is an extension of credit. I will be responsible and liable for all calls billed to my Authorization Code. I will receive an itemized statement and will be responsible to pay the balance due by the due date whether or not I have reached my established credit limit. If payment is not received by the due date, my Authorization Code will be deactivated and I will be subject to a late fee.

3. NON-PAYMENT: If my Authorization Code is deactivated due to non-payment, the balance due must be paid before my code is reactivated. Non-payment of my TS account is treated like any other debt owed to the university. Sanctions such as withholding registration privileges, diplomas, and grade transcripts will be invoked on all past due accounts. Additionally, past due accounts may be turned over to a collection agency.

4. RATES: Domestic Long distance calls will be billed at 1+DDD(direct distance dialing) rates, less 10%. International calls will be billed at DDD. These rates are subject to change.

5. CREDIT LIMITS: A $200.00 credit limit is in effect. This credit limit includes all calls made with my Authorization Code and any additional charges incurred. TS will make every effort to deactivate my code once the credit limit is reached. I do understand that should my balance exceed the credit limit, I will be responsible for the balance in full.

6. BILLING: TS will send an itemized statement after each billing cycle. It is my responsibility to notify TS if I have not received my statement by e-mail.

7. CHANGE OF ADDRESS: If my home address changes or if I leave Radford University, it is my responsibility to notify TS immediately. Failure to do so will result in my continued responsibility for all telephone charges.

8. RETURNED CHECKS: If my check is returned from my bank, I understand the amount of the returned check will be reapplied to my tuition account. I will also be assessed a returned check fee. I understand that my Authorization Code will be deactivated until I replace the returned check and pay the fee. TS will not resubmit any returned checks.

9. TERMINATION OF SERVICE: This service may be canceled at anytime by either party. TS may terminate my service if I fail to abide by the terms and conditions of this agreement. TS also reserves the right to refuse service or require a deposit for any individual considered a credit risk. If my service is terminated, I understand that I will be responsible for the incurred balance through the date of termination.

10. UNAUTHORIZED ON-CAMPUS SERVICES:  Contracting for unauthorized services or accepting collect calls will result in a $10 service fee.   Please note 700 and 900 numbers are restricted from on-campus phones.
